Legal Name Change Information
Students who wish to request a legal name change must submit a Change Name form (available via the SIR section of the Document Library in eForms).
Employees who wish to request a legal name change must submit a Change of Personal Information form (found on the Employee shared drive under Human Resources > Forms for Current Employees).
Certain departments and processes require use of a legal name. These include:
- reporting to state and federal agencies
- federal and state tax forms and reporting, including W2, W4, 1099, 1095C, and 1098T
- immigration and visa records
- international student reporting
- financial aid records
- student accounts, billing, and cashiering records
- medical and health records
- insurance enrollment and other records
- student conduct records
- campus police and security records
- student/employee employment, payroll, and benefits records
- official transcripts
- professional licensures
- course enrollment and degree verification
- National Student Clearinghouse reporting
- transfer and external credit records
- other records where a legal name is required by law or College policy
Non-Legal Last Name Changes
Students who need to change their last name due to extenuating circumstances (e.g., domestic violence), but do not have the ability to legally change their last name, should contact Admissions, Registration, and Records. Employees who need to change their last name due to extenuating circumstances, but do not have the ability to legally change their last name, should contact Human Resources.
